The following information applies to applicants choosing to proceed by mail. If you plan to visit our office in person, please refer to the in-office option for further instructions.

For a new passport application, your documents must first be presented at a designated acceptance facility before being mailed to our office. At this location, you will sign your application in front of an authorized agent, and a standard execution fee will apply.

The agent will place your documents into a sealed envelope—please do not open this envelope after it has been sealed.

There are limited locations available for this step, but our team will assist you in finding a convenient option and guide you through the process.

Please note:
  • Do not sign your application until you are instructed to do so
  • You must provide your Social Security number if one has been issued to you
Our team will guide you through completing your application accurately to help ensure everything is properly prepared before moving forward, reducing the risk of delays.
To apply for a new or first-time passport, you will need to provide an original or certified copy of one of the following
  • Certified birth certificate (long form, not abstract)
  • Consular Report of Birth Abroad
  • Naturalization certificate
Birth Certificate Requirements:
Your long-form birth certificate must:
  • Be issued by the city, county, or state of birth
  • Include your full name, date of birth, and place of birth
  • List your parent(s)’ full names
  • Show the official filing date (within one year of birth)
  • Include the registrar’s signature
  • Display an official seal (embossed, impressed, or multicolored)

If you do not meet these requirements, additional supporting documents may be needed.

If you have previously held a U.S. passport but do not currently have proof of citizenship, a file search request may be required. Our team can guide you through this process.

For Minors (16 years of age or younger):
  • A certified long-form birth certificate is required
  • A previous passport (if available) should also be provided

DS-11 Online Form
Your passport photo must meet the following requirements:
  • Sized at 2″ x 2″
  • Taken against a plain white background
  • Captured within the last one month
We offer professional passport photo services at our office to help ensure your photos meet all required specifications and are ready for submission without delays.
Payment must be made by personal check or money order to the appropriate issuing authority.
  • Adult: $170
  • Child: $140
Our team will guide you on how to correctly prepare your payment to ensure everything is completed accurately and avoid any processing delays.

If you have a copy of your travel itinerary or ticket, please include it with your documents. If you do not have a confirmed itinerary, please contact our team. We will guide you on the next steps and help you proceed smoothly without delays.
Letter of Authorization
Please note:
  • Check only the first two boxes and leave the third box blank
  • Do not fill in the courier company name until you have spoken with our team
You will include one letter of authorization in your package. The second letter of authorization must be sent with your sealed envelope after it has been completed at the acceptance facility.
For minors (16 years of age or younger):
  • Both parents must sign the letters of authorization
Our team will guide you through completing these forms correctly to ensure everything is properly prepared and to help prevent delays.
Service Order Form

If you are submitting your passport application by mail, please complete the service order form and include it with your documents.

Our team will use this information to review your request, organize your paperwork, and guide your application through each step to help ensure a smooth and efficient process.

If your name has changed due to marriage or divorce, you will need to provide a certified copy of your marriage certificate or a legal court document confirming your name change.
