- Prepare Form DS-11, but do not sign it until instructed.
- Gather the child’s citizenship evidence and proof of parental relationship.
- Prepare parent or guardian photo IDs and photocopies.

Service help starts at $199 + government fees + shipping. Government examples for a child passport book include a $100 application fee and $35 acceptance fee. Expedited service is $60 if used.
What if my child is 16 or 17?
