Lost, Stolen or Damaged Passport

For lost, stolen, or damaged passports, you’ll need to appear in person at a passport acceptance facility after preparing your documents. Your application will be sealed in an official envelope, which must remain unopened so we can continue assisting you with the expediting process.

In Person Submission and Fee Details

After preparing your documents, you’ll need to appear in person at a passport acceptance facility to submit your application.

During your visit, you’ll pay the required government fees directly at the facility, including the application fee and execution fee using accepted payment methods.

Your documents will then be placed into an official sealed envelope and returned to you. This envelope must remain unopened and brought back so we can continue assisting you with the expediting process and help keep everything moving forward without delays.

How to Apply

Complete Online Applications

Fill out your application online and ensure all information is accurate to help avoid delays.

Please note:

  • Do not sign the application until instructed to do so
  • If you have been issued a Social Security number, it must be provided as part of your application
  • A DS-64 form must also be completed for lost or stolen passports

You’ll need to provide one valid form of identification as part of your application. Acceptable options include:

  • A current, valid U.S. driver’s license
  • A U.S. passport in good condition, even if expired
  • A Certificate of Naturalization or Citizenship
  • An employee ID issued by a city, state, or federal office
  • A U.S. military or dependent ID
  • A valid foreign passport
  • A Matricula Consular ID (commonly used for child applications)

If you’re unsure which document to provide or don’t have one of the options listed, we assist in guiding you through acceptable alternatives to help avoid delays.

If you previously had a U.S. passport but no longer have proof of citizenship, you may need to request a file search as part of the process.

If available, include a copy of your flight itinerary or travel confirmation with your documents to support your timeline. Don’t have travel plans yet? You can still move forward with your application. We’ll guide you on what to include so your process stays on track.
You’ll need to provide a recent passport photo that meets the required specifications:
  • Size must be 2″ x 2″
  • Taken on a plain white background
  • Captured within the last 30 days

If your passport book is lost but you have a previously issued passport card, include it with your application.

If your name has changed due to marriage or divorce, include a certified marriage certificate or an official court document that confirms your legal name change. The government expedite fee must be paid by cashier’s check or money order, payable to the “U.S. Department of State.”

  • Sameday Passport visa offers clients the ability to pay everything by debit card, cash or check and we will obtain the money order for an additional fee of $10.00.
