Complete Online Application
Fill out your application online and make sure all details are accurate to help avoid delays. Do not sign the application until instructed to do so. If you have been issued a Social Security number, it must be included as part of your application. We assist in reviewing your information to ensure everything is properly completed before moving forward.

Provide an original valid form of identification with your application. Acceptable options include a valid driver’s license, state-issued ID, or military identification.

If you’re unable to provide one of these primary forms, alternative identification options may be available. Additional documentation may also be required in certain situations, such as name or gender updates.

Second Limited Passport Statement

Letter of Authorization
If you’re submitting your application by mail, complete two Letters of Authorization and include them with your documents. We assist in reviewing your paperwork to ensure everything is properly completed and help keep your application moving forward without delays.
Service Order Form

Expedited government fees require payment via cashier’s check or money order addressed to the “U.S. Department of State.”
